1. To Pet Purchase Declaration, the applicant has to approach the Animal & Veterinary Service in Singapore contact link
2. Applicant has to get the form from the respective office or can download the from the following link
3. Applicant has to fil-in all the parts in the application form.
4. Make sure that you have attached all the documents mentioned in the “Required Documents” section of this page.
5. Submit the application form along with the documents to the respective officer.
6. Once the process is completed, the applicant can collect the certificate from the office within the stipulated processing period of time.
