1. To Get Means-Test Declaration Form, the applicant has to approach the Ministry of health link
2. The applicant has to download and complete the application form given in the following link
3. The applicant has to complete all necessary details in the application form.
4. After filling the application form, make sure to attach all the documents listed in the “Required documents” section of this page.
5. The applicant has to pay the prescribed fee as per the authority's request.
6. Submit the application form to the ministry of health. contact details of the office were given in the “Office Locations & Contacts” section of this page.
7. The authority will review the application and contact the applicant via email or SMS.

Eligibility criteria for SMF Assistive Device Subsidy:

• Aged 60 years and above.
• Undergo qualified assessor’s assessment to determine the need and type of device; and
• Household monthly income per person of $2,000 and below OR Annual Value (AV) of residence of $13,000 and below for households with no income.

Eligibility criteria for SMF Consumables Subsidy:

• Aged 60 years and above
• Receiving home-based healthcare services or home palliative care services
• Household monthly income per person^ of $2,000 and below OR Annual Value (AV) of residence of $13,000 and below for households with no income and
• No other sources of concurrent subsidy for similar consumables.

• This form is used for patients/ clients to undergo household means-testing1 for application for various government subsidy schemes (see descriptions below). Besides means-testing, patients/ clients will still need to meet all other eligibility criteria to qualify for any schemes. Patients/clients do not need to complete this form if they have already been means-tested through any of these schemes in the past two years.
