Apply In Person

1. To Apply Authorization For An Embryologist, the applicant has to approach the Ministry of Health link
2. An application form will be provided to the applicant by the respective officer.
3. The applicant has to fill in all the necessary details in the application form.
4. After filling the application form, make sure to attach all the documents listed in the “Required documents” section of this page.
5. Submit the completed form with the necessary documents to the respective officer.
6. The applicant has to pay the prescribed fee as per the authority request and submit the receipt as proof of payment.
7. Upon successful verification, the authorization certificate will be issued within 15-30 days.

• Able to work on Sundays and Public Holidays and should be willing to work outside office hours whenever required.
• Possess good communication and patient-relations skills.

• The fee for this procedure has to be paid as per the authority's request.

• The validity of this procedure is for 3 years.
