1. Apply to obtain a replacement card at the ID Office located on the ground floor of the Oceangate House.
2. Complete and submit an application.
3. You will also need to submit the following:
   1. In case you lost your identity card, you will need to present police report when you apply for the procedure.
   2. As for the case of disfigured ID card, the application must be accompanied by the old ID card.
4. If you have changed your name, you must notify the Chief Officer of the Civil Status within 60 days of the change. The application for a new card must be accompanied by the old card and a copy of the authorization of change of name issued by the Civil Status.
5. Pay the replacement fee.

• Completed application
• In case you lost your identity card, you will need to present police report when applying for a replacement.
