1. To obtain an International Vaccination Record, applicants will need to go to a public health facility.
2. Applicants will be asked to present their national identity card or former international vaccination record if it is available.
   In case of unavailability of the book in public health facilities, an applicant must acquire a notebook of vaccination from private dealers.
3. Vaccination and issuance of the record is free of charge if you apply at a public health facility.
4. The Vaccination record will be issued to you immediately.

Note: In case you have lost your vaccination record, you will need to follow the same procedure to obtain a new one.

Anyone wishing to travel to a foreign country may obtain an international vaccination card.

Vaccination is free at public health facilities so is the vaccination record.
