1. Obtain your Certificate of Motor Disability from any public and private health facilities or doctors with knowledge or expertise in the field of disability.
2. Submit a request and present your valid identification.
3. Pay the issuance fee.
4. Once the doctor or the physician has confirmed the applicant's condition the certificate of motor disability will be issued immediately.

The amount charge for the issuance of the certificate of disability varies depending on the facility or the doctor issuing the certificate.
