Apply In-Person

1. To apply for a Custom Certificate, the person concerned should visit the nearest/any civil status center and file the process. (Please view contact and address details below).
2. Express your interest to the registrar, and submit the request letter, attached to the supporting documents (mentioned under required documents section below) to the registrar to be validated.
3. The registrar will then verify, if all the necessary information and documents have been provided and that the applications comply with the selection criteria, if everything is in order, you will then be required to pay for a sticker.
4. The registrar will then draw-up the certificate on plain paper, sign, and stamp it, the certificate is then issued to the applicant immediately.
