Apply In-Person

1. To apply for a Collective Life Certificate, the person concerned should visit the nearest civil status center office. (Please view contact and address details below).
2. Express your interest to the registrar, submit the supporting documents (mentioned under required documents section below) to the registrar to be validated.
3. The registrar will then verify, if all the necessary information and documents have been provided and that the applications comply with the selection criteria, if everything is in order, you will then be required to pay for a sticker, whose value varies according to the localities.
4. The registrar will then draw up the certificate of collective life, sign, and stamp it and issue it to the applicant immediately.

• The father, mother, or guardian of a minor child are eligible to apply.

You must pay for a sticker whose value varies according to the localities:

• 200 to 500 FCFA in the municipalities
• 75 to 150 FCFA in rural communities
