• Letter from the employer or his deputy, must be sealed and certified by a Chamber of Commerce; and if the sponsor could not attended in person at the date, location, number, date and source of the lost passport, and that the passport is not reserved or in custody of any government body
• Declaration of the lost passport in one of the local newspapers
• Copy of the lost passport, if any
• Copy of the residence permit of the lost passport holder.
• Case investigation documents filled by the passport holder and stamp them with the seal of the enterprise or the employer
