Ordering Online
- Make sure that you have a valid credit card before you begin the process.
- Be ready with the following information:
- Applicant
- Birth Subject
- Mother
- Father
- Fill in all the required information on the Online Certificate Ordering Window.
- Your online order must be completed in one session. You cannot save it on your computer, if you stop typing for more than 30 minutes, your order will time out and all information entered will be erased.
- Scan and attach an authorization form in case the applicant is not eligible to apply for a commemorative stillbirth certificate. A screen will be provided during ordering process to allow you to search your computer for the signed authorization information and attach the file.
- Print your payment receipt, you will need the Order number indicated on the receipt when you communicate with Saskatchewan Vital Statistics.
Manual Processing
- Download and fill up one of the following documents:
- Submit the completed form and payment:
- Fax: 306-787-2288 (All fax orders must be paid by credit card)
- Mail: Send your request to, eHealth Saskatchewan, Vital Statistics Registry, 2130 11th Avenue, Regina, SK S4P 0J5. Mail orders can be paid by money order, cheque or credit card. Please make money orders and cheques payable to eHealth Saskatchewan. Money orders must be in Canadian Dollars.
- In person: Submit your request at 2130 11th Avenue in Regina, SK

For manual processing:
- A copy of one piece of government-issued identification with photo, or
- Copies of two other pieces of identification - one must have your signature on it.
- Completed request form.

Eligibility
Individuals who may request a commemorative stillbirth certificate:
- The mother, father or any other parent as listed on the original registration of stillbirth.
- The adult sister or brother of the stillbirth infant.
- A person authorized in writing by a parent or sibling of the stillborn child.
- The personal or property guardian of a parent or sibling of the stillborn child. Must be able to present a Court Order or other legal documents.
- A Saskatchewan Court order or subpoena can be presented by an individual to legally request a certificate, copy, or extract from any Vital event.
Copies of the registration and medical certificate of stillbirth can be issued to the following:
- The doctor or coroner that signed the original certificate of stillbirth
- The coroner or medical health official conducting an investigation or a program to control a disease.
- A person applying for a disinterment permit as required by any Act or regulation
- The Police, during investigation:
- municipal, provincial, territorial or RCMP
- Department of Citizenship and Immigration
- Canadian Security Intelligence Service (CSIS)
- Canadian Border Services Agency (CBSA)

Commemorative Stillbirth Certificate: $25
Note:
- If you are paying by credit card please complete the Payment Information Form and attach to the application.
- All fax orders must be paid by credit card.
- Mail orders can be paid by money order, cheque or credit card. Please make money orders and cheques payable to eHealth Saskatchewan. Money orders must be in Canadian Dollars.
Validity

Processing Time
For manual processing you may receive your Vital Statistics Certificates within 12 business days or 5 business days if you availed of the Priority Service.

Instructions
You may obtain a commemorative stillbirth certificate by ordering it online or personally request for the document.
Ordering documents online is much faster and easier, fees are paid using a credit card. Those who do not have a valid credit card must go through the manual process of submitting their request.
