1. To apply for a change of name in Saskatchewan, the applicant will need to contact eHealth Saskatchewan, use the following Link

4. Once the applicant contacts them, eHealth will provide further information on what the applicant will need to do.
5. The applicant will need to fill out an application form and attach all the documents as mentioned under the "Required Documents" section of this page.
6. The applicant will also need to give a criminal record check.
7. Submit the application form as directed by the authorities.
8. The applicant will receive their change of name certificate after the submission of the application.

• The applicant applies for a change of name if they are 14 years of age or over and domiciled in Saskatchewan.
• The applicant can apply for a change of name for any reason other than marriage, annulment, death of spouse or divorce.

• Information will be given by the authorities.

• The document is valid forever and does not need to be renewed.

• The applicant will obtain the application form by calling the number under the “Office Location and Contacts” section of this page.

• The applicant shall receive their certificate within a few weeks to a month from the submission of the application.

• The applicant does not need to apply for a change of name if they are changing their last name after marriage, correcting an error in their birth record or if the applicant is changing parentage.
• The applicant does not need to change their name because of marriage, divorce, the marriage being annulled or the death of the spouse.
