1. Have your documents authenticate by submit and paying for notary fee services via the IREMBO Government portal via this link: IREMBO and in the search box type the word Notary and search.
2. Upon a successful search, click on Notary services to open a notification tile read the information and then click on APPLY to open the application from
3. Fill the application form with the correct details and hit on the NEXT button to go to the summary page, select one of the option provided for feedback and declare that you have provided the correct information and then submit to go to the payment page, Pay the recommended fee using one of the provided options and go to the notary office you select you have your documents notarized. Note that the documents to be notarize are : License to practice pharmacy profession and Pharmacy degree
4. Once you have the notarized the document apply for the pharmaceutical wholesale license by:
5. Paying the license fee at a bank using the details provide in “Fee” section below and obtain the payment receipt
   • Obtain the application form from the FDA office in your location and file it accordingly or download it directly via the link provide in “Documents to Use” section below
   • Prepare your application document as mentioned in the application form and submit them to the Rwanda Food and Drugs Authority (FAD) via email to this address: info@rwandafda.gov.rw or in person at the FAD office.
6. The FAD officer at the reception will receive your application, verify it and if it is correct and forward the application for assessment and approval
7. Then the FAD will send an inspector to conduct inspection on your business premises focusing on location, construction standards, maintenance and sanitation of the wholesale pharmacy and areas of sales and storage. NOTE: All records related to pharmaceuticals products and staff should be kept for at least 10 years.
8. Once the inspection is compete, the inspect will file a report of the inspection results to the FAD
9. Upon a success application and inspection, the FAD will prepare your license and email a scanned copy to you
10. Then visit the FDA office, Pharmacovigilance and Food Safety Monitoring unit to collect the original Pharmaceutical Wholesale License in person

For the company

• Application form wholesale pharmacy, the form must bear the signature of the proprietor, or that of the person officially representing a given pharmaceutical institution, or one with power to sign on behalf of the institution. The form must be approved by the respective Mayor of the District.
• Payment receipt
• Certificate of domestic company registration
• Lease contract
• Architectural plan of the warehouse

For the responsible pharmacist

• License to practice pharmacy profession (an authentic copy)
• Pharmacy degree (authentic copy)
• Curriculum vitae
• Employment contract
• Written commitment to not practice cumulative medicine – pharmacy (original)

For the owner and the responsible pharmacist

• Proof of identity
• Passport size photo
