Apply In-Person:

  1. To Report Lost or Stolen Passport, the applicant has to approach the Australian diplomatic or consular mission. Contact details are mentioned in “Office Locations & Contacts” of this page. Or use the following given Contact Link
  2. An application form is available at the following Link
  3. The applicant has to fill-in all the details in the form.
  4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
  5. Submit the application with the required document to appropriate office.
  6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
  7. There is no fee required for this procedure.
  8. Government of Australia will issue the certificate and notify the applicant.
  9. Please contact Government of Australia for further details.


Apply Through Call

  1. To Report a Damaged Passport, the applicant has to approach the Australian Post or the Australian diplomatic or consular mission and collect the application form and apply as same way for a first-time passport. Contact details are mentioned in “Office Locations & Contacts” of this page. Or use the following given Contact Link
  2. Call Government of Australia Department of Foreign Affairs and Trade if the applicant unsure of their passport's condition.
  3. The applicant has to fill-in all the details in the form.
  4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
  5. Submit the application with the required document to appropriate office.
  6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
  7. The fee of this procedure is mentioned under the “Fees” section of this page.
  8. Government of Australia will issue the certificate and notify the applicant via Email or SMS.

Need for the Document

  • People who fail to notify the loss or theft of their passport as soon as feasible may face consequences under the Australian Passports Act of 2005.
  • Making false or deceptive remarks is also against the law.
  • Up to ten years in prison, a fine of one thousand penalty units, or both are possible penalties.
