To Obtain Proof of Age Card, the applicant has to approach the transport and motoring customer service centre, participating Queensland Government Agency Program Office (QGAP), or a police station that provides licensing services. The information needed to be filled in the application form are listed below:

• Personal details
• Applicant Status
• QLD driver licence, industry authority, marine licence indicator, APA card or Photo ID card details
• Name difference
• Applicant’s declaration

• For Queenslanders who are 15 years of age or older, a photo identity card that serves as age verification is an acceptable alternative to a driver's licence or passport.

• Adult proof of age cards that were issued before April 2019 should still be accepted until the card's stated expiration date on the front of the card.

• Proof of Age card can be used as proof of identity.
