Apply Online

1. To update personal information of healthcare practitioner through online, visit the following link: Online portal
2. If you already have an account, please enter your username and password then click on “LOGIN” button.
3. If you don’t have an account, click on “REGISTER” button.
4. Now complete the form with necessary information then click on “Submit” button.
5. Now you will receive an email with the instructions. In the Email please click on “Activate My Account” button.
6. In the next page, please enter the new password and click on “Submit” button.
7. After successful login, click on “Create and Submit Request” option then click on “Apply for Restoration to the Registry” option.
8. After successful login, click on “Create and Submit Request” option then click on “Apply to Update Personal Information” option.

   • Note: “Apply to Update Personal Information” option available only for license is applied in the account.

9. Now complete the form with necessary information then you can upload the required documents mentioned in the required documents section below then click on applicable button.
10. Once you complete this, you will be redirected to a screen in which you can review your information.
11. After reviewing your information, click on applicable button to submit your request.
12. Now the authorities will verify your application and validate the submitted documents.
13. After verification process, personal information of healthcare practitioner will be updated.

• Copy of valid passport.
• Copy of valid QID card (front and back).
• Healthcare practitioner registration.

• Health practitioners who have valid Healthcare practitioner registration can update.

• Pay the fee amount displayed on the screen during online application process.

• The processing time is between three to five days. This time may vary depending on the availability and correctness of the information provided.

• Health practitioners shall submit a request to the Qatar council for healthcare practitioner at Ministry of Public Health (MOPH) to update personal information of healthcare practitioner.

• Name of the applicant.
• Address.
• Phone number.
• Email ID.
• Qatar ID card number.
• Passport number.

• To update the personal information in their medical profile.

• The Ministry has the authority to abandon the candidate or take any official action against forgery or manipulation of data by the applicant.
