Step 1: Obtain a Police Report
- You should start by visiting a police station to report the loss or damage of the health card. You are to acquire a police report from the police authorities which is a prerequisite to making the application.
- Attach this police report with your application.
Step 2: Apply In-Person.
- Submitting a request to reprint a lost or damaged health card is a simple process and can be done immediately at all Hamad Medical Corporation health centers and hospitals.
- The applicant should visit the nearest Primary Health Care Centre (PHCC) nearest to where you live. Locations of PHCC can be obtained from the following link.
- Once there ask the Patient and Visitor Service Center staff to direct you to the right location in the PHCC to make the replacement application.
- There you will be provided with appropriate forms which you will need to fill out the and submit with all the required documents mentioned in the “Required Documents” Section of this page. These include:-
- Pieces of damaged health card (if applicable)
- Copy of expired health card if available.
- Police report of the lost/ damaged license
- Passport or ID copy
- Passport size photographs (two)
- Residency permit (RP)
- Qatar ID (issued when you get your RP)
- The lost/ damaged health card replacement application along with your documents can be submitted to the PHCC offices only from 7.30am until 2pm on weekdays.
- The PHCC officials will go through your replacement application and ask you to make the appropriate payment.
- Make the appropriate replacement application payment via debit/credit card. No other form of payment is accepted. If you don’t have your own, you can bring a friend or relative along and use theirs.
- PHCC can give you the replaced health card the same day or will ask you to come any other day to collect the new replaced health card.
Apply Online
- You should start by visiting a police station to report the loss or damage of the health card. You are to acquire a police report from the police authorities which is a prerequisite to making the application.
- You must first register with the website before you can avail of this service. To register follow link: Qatar e –Government portal
- Load the online application page through the following link
- Click on the Apply online tab to access and fill out the online form.
- Make sure you enter all the required information. Enter your Qatari ID number, then select “Replace (Lost/Damaged health Expiry Date)” in the “Card Information” page, then click on “Next”.
- Select the number of years of validity you wish your card to have under the “Number of Required Years” option and choose your preferred delivery option , then enter telephone number in the “Application Form” page.
- Choose your preferred delivery option. You may collect the card from Rumailah Hospital for free, or pay a fee to collect it from a post office, or have the card delivered.
- Enter your email if you selected “Yes” to receive the receipt voucher by email.
- Enter cell phone number if you select “Yes” to receive SMS notification when the card is ready.
- Review the application for renewal information to ascertain that the information to ensure that everything is okay.
- If everything is okay you can move on to pay the required fees. This is done by Clicking on the “Next” tab to move to the “Payment Details” page, then click on “Pay” to make the payment.
- Pay appropriate fees using a debit or credit card and submit your application for card replacement.

Eligibility
- Qatari citizens, GCC residents, non-GCC residents, visitors and domestic workers of Qataris who have lost / damaged their valid health cards in Qatar.
Fees
- If you lose your health card but it’s still valid, you’ll have to pay QR100 for a replacement for Qataris and GCC residents and QR200 for non GCC Residents and QR100 for domestic staff.
- If you lose it and it has expired, you’ll have to pay double for a new health card. You’ll have to pay QR200 for a replacement for Qataris and GCC residents and QR400 for non GCC Residents and QR200 for domestic staff .
Validity
- The card is valid for a period of one year for expatriates.
- The validity period varies depending with the choice of the applicant.
Documents to Use
- The replacement application forms are provided at the Primary Health Care Centre when you visit them
- Application can also be done online.

Processing Time
- 24 hours if the application is done in Primary Health Care Centre
- If the renewal application is done online or at the post office, the card will be delivered within two to three weeks of submitting the application.

Need for the Document
- Healthcare is provided for free for all citizens and residents of the country via Hamad Medical Corporation to individuals who have the health card. Therefore one needs to have a valid card at all times to be able to access these services.
- In the absence of a good health insurance plan, you will need to turn to Hamad General Hospital and Primary Healthcare Centres (PHCC) for all your medical needs.
- From consultations and blood tests to surgeries and medication, including for mental health, everything is free of charge or heavily subsidised in Hamad and PHCC. The Hamad health card will have your ID number, nationality, photo and date of issue.
- In order to access these services from Hamad Medical Corporation, you’ll need a health card
- Qatar has a heavily subsidized healthcare system. Expatriates, citizens and even visitors can take advantage of this system, which includes a network of primary care centers and clinics offering medical, dental and optometry services for the entire family.
- Qatar's private healthcare services on the other hand are more expensive than those run by the state and not all companies offer their employees’ health insurance. Additionally, private healthcare does not offer a full range of services, whereas the public hospitals—Hamad Medical Corporation (HMC)—do.
