Apply In-Person:

1. To replace duplicate of lost educational certificate, applicant has to visit the Equivalency Affairs Section at the Ministry of education and higher education (MOEHE) office.

3. Applicant has to approach the authorities of Ministry of education and higher education and obtain the application form from the respective officer.
4. Now complete the form with necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the required documents section below with the completed application form.
6. Check the application form and then submit it to the respective officer in equivalency affairs section in the Ministry of education and higher education address mentioned in the office locations & contacts section below.
7. Now the authorities will verify your application and validate the submitted documents.
8. After verification process, the applicant will be later requested to collect the duplicate educational certificate on a specified date.

• Applicant shall submit a request to the Equivalency Affairs Section at the Ministry of education and higher education to obtain a replacement of a lost educational certificate.

• Name of the applicant.
• Contact number
• E mail ID
• Qatar ID card number.
• Passport number.
• Lost certificate number (If known)

• Educational certificate is essential to apply for employment/higher studies. So, a duplicate certificate needs to be obtained if the certificate is lost or damaged.

• The Ministry has the authority to abandon the candidate or take any official action against forgery or manipulation of data by the applicant.
