• The report of a death, including delivery of a still-born fetus after twenty-eight weeks of pregnancy, shall be made to the competent health office within forty-eight hours of such death or delivery.
  

Apply In-Person in Qatar

1. In the event of a death in Qatar, police should be notified as soon as possible, so that they can issue a report stating that there was no foul play involved.
2. Make sure you are legible to report and register the death before beginning the process. The eligibility criteria for persons who can register a death in Qatar are mentioned in the “Eligibility” section of this page. Make sure you are eligible to make the registration.
3. Present yourself to the nearest police station in the area which the death has occurred. This should be done immediately after the death of a person for whom you registering occurs, or not later than 48 hours
4. Give particulars of the death to the best of your knowledge.
5. Once this report has been issued, you need to go to the mortuary to collect the Notification of Death certificate. Bodies are usually sent to the Hamad Hospital Mortuary, where they are held until all necessary documentation is received for repatriation or burial (cremation is not permitted)
6. You then need to visit the the Births and Deaths Record Department at the Ministry of Public Health to make the application for death registration. The Ministry of Public Health MOPH which is the authority responsible for registering Qatari deaths in Qatar.
7. Here the applicant will be issued with a Death Registration Form to fill and submit . The form is available at the Birth and Registration Offices or can be downloaded through this link: Death registration form
8. Fill in the required information on the application form and attach all the required documents to the application form and make the submission
9. These required documents are mentioned in the “Required Documents” section of this page. This include

   • Original Passport / ID of the Deceased.
   • Original ID / Passport of the Informant.
   • Notification of Death issued by the hospital

10. The officers of the birth and registration office will register the death immediately and the informant can now proceed to apply for a death certificate.
    
    

Applying from outside Qatar

1. In the event of a death in Qatar, police should be notified, as should the embassy or consulate of the deceased.
2. Visit the nearest police station to report the death to determine if there is foul play surrounding the death.
3. You should then proceed to visit the nearest Qatari mission or embassy broad to make the application for registering the death.

• The following persons shall be authorized to report deaths:

1. Father of the deceased, if present.
2. Attending adult relatives, males first, then females, who are the closest relative to the deceased,
3. Adult male or female residing with the deceased.
4. The physician who examined the deceased.
5. Managers of hospitals, hotels, jails and other locations where the death occurred.

• The abovementioned persons shall not be responsible for reporting where the sequential order required for persons making the report cannot be established. No unauthorized persons may report a death.

• The death is registration is valid forever once done.

• The Births and Deaths Record Department at the Ministry of Public Health (MOPH) undertakes registering Qatari deaths inside and outside Qatar and issuing of their death certificates, issuing burial authorization for non-Qatari population, sending dead bodies outside the country in addition to other matters.
• In the event of a death in Qatar, police should be notified, as should the embassy or consulate of the deceased.
• Bodies are usually sent to the Hamad Hospital Mortuary, where they are held until all necessary documentation is received for repatriation or burial (cremation is not permitted).
• There is no charge for the mortuary service at the Hamad Hospital.
• It is possible for bodies to be embalmed at the mortuary.

• Death registration is the first step in obtaining a death certificate.
• Registration documents the cause of death, location of death, time of death and some other personal information about the deceased.

• The process should be commenced as soon as death is confirmed, and should not be later than 48 hours.

• Registration is a requisite to obtain official documents which include a death certificate, No Objection Letter to Release Remains and Transfer of Remains Certificate.

• Any person who discovers a corpse shall promptly report the matter to the nearest police station. A coroner or the designated health physician shall be delegated to examine the corpse, determine the cause of death and estimate the age of the deceased. Where any suspicion as to the cause of death arises, an autopsy shall be conducted.
• A record shall be prepared on the circumstances and conditions surrounding the discovery of the corpse. Where the deceased is unknown, the said record shall include an accurate description of the corpse and any distinguishing marks. The corpse shall not be buried without the consent of the investigating authorities.
• The aforesaid record shall be sent to the competent health office in order to enter the deceased in the deaths register. The number and date of the record shall be entered in the space provided for remarks. Where the deceased is unknown, the word “UNKNOWN” shall be entered in the space specified for the name of the deceased.
