Qatar - Apply for Personal Radiation Medical License


Procedure

Apply In-Person:

  1. To apply for personal radiation medical license, applicant has to visit the Qatar Council for Healthcare Practitioners at Ministry of Public Health (MOPH) office.

  3. Application form can be downloaded from the below link: Application form.
  4. Now complete the form with necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
  5. After filling the form, attach the documents mentioned in the required documents section below with the completed application form.
  6. Check the application form and then submit it to the respective officer in the Qatar Council for Healthcare Practitioners in the MOPH office. The address is mentioned in the office locations & contacts section below.
  7. Now the authorities will verify your application and validate the submitted documents.
  8. After the verification process, personal radiation medical license will be issued.




Required Documents

  • Copy of Qatar ID card.
  • Copy of passport.
  • Two copies of valid medical license or copy of valid evaluation.
  • Copy of bachelor degree or its equivalent and copy of the specialization certificate or its equivalent.
  • Updated Curriculum Vitae (C.V.)
  • Recommendation letters (if any).
