Apply In-Person:

1. To apply for medical or surgical privileges, applicant has to visit the Qatar Council for Healthcare Practitioners at Ministry of Public Health (MOPH) office.

3. Application form can be downloaded from the below link: Application form.
4. Now complete the form with necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the required documents section below with the completed application form.
6. Check the application form and then submit it to the respective officer in the Qatar Council for Healthcare Practitioners in the MOPH office. The address is mentioned in the office locations & contacts section below.
7. Now the authorities will verify your application and validate the submitted documents.
8. After the verification process, medical or surgical privileges will be approved.

• Copy of Qatar ID card.
• Copy of passport.
• Surgical/ Medical Privileges Form for the specific specialty (marking in the requested column) and signed by both the physician and the medical director of the facility.
• Two copies of valid medical license or copy of valid evaluation.
• Request letter (cover letter) signed and stamped by the medical director of the place of work, specifying the start date of work, good standing and the name of the facility where the applicant wants to perform the surgeries.
• Request Letter from the facility where the applicant wants to perform the surgeries (it could be in a different workplace from the one mentioned in his/her license).
• Stamped approval letter from the facility where the applicant wants to perform the surgeries (available on the website).
• Copy of official surgical log book for the last three years (must be attested by the official competent authorities).
• Personal Declaration for Surgical/ Medical Privileges.
• Copy of bachelor degree or its equivalent and copy of the specialization certificate or its equivalent.
• Updated Curriculum Vitae (C.V.)
• Recommendation letters (if any).

• Specialist physicians shall submit a request to the Qatar Council for Healthcare Practitioners at Ministry of Public Health (MOPH) to apply for medical or surgical privileges.

• Name of the applicant.
• Address.
• Phone number.

• Qatar ID card number.
• Medical license number.
• Passport number.

• The Ministry has the authority to abandon the candidate or take any official action against forgery or manipulation of data by the applicant.
