Apply In-Person:

1. To apply for dental privileges, dentists has to visit the Ministry of Public Health (MOPH) office.

3. Application form can be downloaded from the below link: Application form.
4. Now complete the form with necessary information and make sure you have all the documents in hand that are listed under the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the required documents section below with the completed application form.
6. Check the application form and then submit it to the respective officer in the MOPH office. The address is mentioned in the office locations & contacts section below.
7. Now the authorities will verify your application and validate the submitted documents.
8. After the verification process, dental privileges will be approved.

• Copy of Qatar ID card.
• Request letter from the place of work mentioning the requested privilege.
• Copy of work experience certificate in the request privilege.
• Copy of training certificate/ courses attended in the requested privilege.
• Curriculum Vitae (C.V.)
• Copy of valid medical license.
• Personal Declaration of dental privileges.
• Copy of bachelor’s degree or its equivalent.
• Copy of post graduate degree certificates.
• Any other additional requirements requested.

• Dentists shall submit a request to the Ministry of Public Health (MOPH) to apply for dental privileges.

• Name of the applicant.
• Address.
• Phone number.
• Email ID.
• Qatar ID card number.
• Medical license number

• Dental privileges is needed to do dental treatment under general anesthesia.

• The Ministry has the authority to abandon the candidate or take any official action against forgery or manipulation of data by the applicant.
