Apply In-Person:

1. Application form for renewal of registering as a veterinary practitioner in Animal Husbandry and Animal Welfare Department Puducherry.

3. After getting the application form, the applicant has to fill in all the necessary details.
4. Applicant should make sure to carry the originals and photocopies of the documents that are listed under the “Required Documents” section of this page.
5. Applicant has to attach the photocopies of the documents (whichever is necessary) along with the application form while submitting it at the office.
6. Fee must be paid as per the authority’s request.
7. Renewal of registration will be done within 15 days from the day of submitting the application.
8. If the renewal has not been done within a month, then, the applicant has to visit the office to know status of their application.

• Lid Registration ID Card (Original)

• Applicant should follow all the instructions provided by the authority.
• Applicant must keep track of their date of renewal.

• Nationality
• Address
• Date of Birth
• E-mail ID
• Employment status with detail
• Basic veterinary qualifications possessed by the applicant
• Registration Number

• Renewal of registration must be done for continuing the veterinary practicing for another year.
