Prince Edward Island - Obtain a Death Certificate


Procedure

  • Go to the nearest Access PEI Centers.
  • Present the required documents.
  • Fill out an application form. (An application form is available online. Check the link below.)
  • Pass accomplished forms. Present other documents or proofs of identity if necessary or when asked.
  • Pay through certified or accredited payment channels.
  • Pick up the certificate or wait for delivery.
  • Present valid IDs (and authorization letter if necessary) in claiming/receiving the certificate.

Eligibility

  • Any individual who has provided sufficient information to the director may apply for a certificate in an arranged form in respect of the registration of death.
  • No certificate or record issued shall divulge the cause of death.

Processing Time

It usually takes a week. If you want to expedite it, you may opt to avail the rush service, which takes 2 working days. The applicant shall shoulder courier expense.

Instructions

  • Prepare important documents, such as birth and marriage certificates of the deceased. These will most likely be asked to the applicant to present.
  • Bring/Present valid proofs of identity.
  • Seek for an authorization letter from the applicant (if only asked to pick up/claim) and present it when claiming.




Required Information

  • Full name of the deceased
  • Gender
  • Marital status
  • Date (mm/dd/yy) and place of death
  • Age and date of birth (mm/dd/yy)
  • Residence prior to death
  • The applicants name, address, and telephone number
  • Relationship to the person whose name appears on the certificate and reason for applying a certificate
  • Mode of payment: money order, debit card, visa (expiry date)





Need for the Document

A death certificate is a document which contains particulars relating to a dead person, including the name of the individual, the date of his/her birth, and the date of his/her death. More so, it is issued by a medical practitioner affirming the death of a person and its cause.

  • Any individual who wish to apply for a death certificate should present a government-issued ID.
  • Present a valid ID when claiming a certificate. If an individual other than the applicant will pick up or claim a certificate, he/she must present an authorization letter with valid ID.
  • Certificate can be delivered at the applicants expense.
