• Go to the nearest Access PEI Centers.
• Present the required documents.
• Fill out forms. (An application form is available online. Check the link below.)
• Pass accomplished forms.
• Pay corresponding fees through accredited payment systems.
• Present valid IDs (and authorization) in claiming/receiving the certificate.

• A minors guardian, who has a lawful custody of a minor (who have resided in PEI for at least three months), may apply for a name change in behalf of the minor applicant.
• A written consent from the lawful guardian or parent who has lawfully accessed to the minor whose name is to be changed must be provided.
• The consent must be personally written by the minor whose age is 12 or older.

• Applicants (person whose name is to be changed) present and proposed name in full
• Date and place of birth (person whose name is to be changed)
• Details regarding the persons (person whose name is to be changed) previous change of name
• Applicants full name, address, place of ordinary residence during the application, length of stay in the province
