- Parents, relatives or legal representatives of children born in facilities not able to register births must visit the civil registry office within days
- They should possess their legal identification documents
- They should state the name of the child and the date of birth and names and id no of the parent(s) and where the baby was born

Instructions
- Birth occurred in Portuguese territory must be reported verbally within 20 days in the civil registry or, if the birth occurs in the health unit where it is possible to register directly.
- Birth registration gives all children the right to know who their mother is and, hopefully, their father too. According to the UN's Convention on the Rights of the Child, children should have the right - as far as possible - to know both their parents.
- Registering your baby also helps the authorities maintain up to date records about the local population
- For unmarried fathers, signing the birth certificate gives you the same legal status in relation to our child as that of a married father: this is called "Parental Responsibility".
- Registration can often take place in the hospital before the mother and baby go home; otherwise, you'll have to visit your local register office.
- The following people are authorized to register the birth:
- The parents
- Parents' legal representative (legally empowered)
- A close relative
- The director (or any appointed administrative staff member) of the place of birth
Required Information
- Name of the child
- Date of birth
- Names of the parents
- City and province of birth
- The date, name and signature of the certifying Registrar, Acting Registrar or Secretary, and the office seal.
Need for the Document
The registration of the birth provides your child with their birth certificate which theyll need throughout their life for example, to gain a passport or for you to obtain child benefit for them.
As soon as a baby's birth is registered it has a legally binding name. Under Portuguese law, the complete name may not exceed six individual names (a composite of a maximum of two first names and four surnames).
Paternity in Portugal is established, unless otherwise stated, through marriage. This means that if the parents are married, either one may register the child. When the parents are not married, both need to go to the Registry Office so that the father's name can be added to the birth certificate.
