Following procedure provides information about applying In-Person
Step 1 : You have to get in touch with the local DMV and enroll yourself as a student who wish to learn driving. When yoou have enrolled yourself, you will be provided with a PKK Number (Profile Trainee Driver), which will be used all along the way to track.
Step 2 : With the PKK number you have to find a local driving school who should be able to help you improve your driving skills.
Step 3 : Before going for the driver test, you would have to get in touch with a doctor who will certify you whether you are qualified for driving. If the doctor says it's okay for you to drive, you could will be provided with a certificate saying so. (Medical Certificate).
Step 4 : Make sure you have all the required documents that are listed under the Required Documents section of this page.
Step 5 : To find out where to apply, please look under the Office Locations & Contacts section of this page which could help.
Step 6 : After the application is sumitted your application will be validated, if everything is satisfying, you will be called for a pratical driving test and a theory test. For the pratical driving and theory tests, you will be trained in the driving school with which you have enrolled.
Step 7 : If you have cleared the driving theory and pratical exams you will be informed by displaying you Social Security Number through the following page : Driving Test Results
Step 8 : You will be requested to pay a fees, if you are successful with the above steps. After 9 days from the date on which the fees has been paid, you will receive you driver license.
Step 9 : In cases where you have to go in-person to get your driver license, you will be requested to bring some form or identification to identify yourself before getting a driver license.

Following procedure provides information about applying through internet

1. To apply for a driver license on-line you will have to have profile trusted ePUAP or secure electronic signature.
2. You would have to follow the same steps as above, but you could send the application through ePUAP.
3. You will have to login to ePUAP and locate servie Obtaining a License, fill out the necessary application form and attach the necessary documents by scanning them. All these documents that are being scanned and attached with the application should be produced in original for verification (probably should be mailed).

5. After the application is submitted your application will be validated, if everything is satisfying, you will be called for a pratical driving test and a theory test. For the piratical driving and theory tests, you will be trained in the driving school with which you have enrolled.

• ID Card or Passport - If you are a foreigner take your passport or residence permit.
• A document that confirms registration address, such as ID card, residence card or certificate of arrival for permanent residence or temporary. If you have a new ID card, which does not have a registered address, take any municipal office regarding the registration certificate.
• Photocopying current driving license - if you already have a driver's license and want to get right to the next category.
• Written consent from parents or legal guardians - if you're 17 years old and soon you finish 18 years. The document parent or legal guardian must agree to the start of the course and issuing a driver's license. Remember that your parent or legal guardian must sign a document at the official. So it is best go to the office with your parent or legal guardian.
• Psychological judgment about the lack of psychological contraindications to drive the vehicle - if you want to get a driving license of category C1, C1 + E, C, C + E, D1, D1 + E, D and D + E, or when the governor turned you on psychological research.
  
  

Description of photo, how should it be :

• It should have your current appearance
• The picture should be clear.
• The picture should be colorful
• Dimenstions : 3.5 x 4.5 cm
• The picture should show the person without hat and dark glasses. The head must be positioned so that it can be seen with visible left semi left ear. The face must be evenly illuminated.

Note :

1. If you have a congenital or acquired defects of vision, you can make a photograph in dark glasses. However, similar photo - glasses - you need to have on the identity document,
2. If you wear a head covering in accordance with the religion we profess - you can be in the photo, if a face is still visible. However, similar photo - of headgear - you need to have on the identity document.
   

If the photo didn't meet the above mentioned criteria, the picture will be rejected.

Office Locations where applications should be submitted'
In the office of the district office in your area. If you live in a city with county rights - apply now the city council. If you live in Warsaw - submit an application to the appropriate district office for your place of residence. The best go to the office in person. It may happen that due to illness, disability or other reason, you can not personally go to the office. Then you can send all the documents by post. If you are a diplomat - submit an application in the Office of the Marshal Mazowiecki in Warsaw.
