Philippines - Obtain a copy of Death Certificate


Procedure

Apply Online:

  1. To Obtain a copy of Death Certificate, the applicant has to make the application online via Philippines Statistics authority (PSA) online portal or tap the following Link
  2. The applicant has to click on “Request Now” then read and accept the terms and condition check box and click “Next”.
  3. Next the applicant has to answer the questions displayed on the screen and click “Next”
  4. The applicant has to fill in all the necessary details in the application form.
  5. Then the applicant has to fill in the delivery details and click “Next”.
  6. Review the application summary and click “Submit”
  7. An acknowledgment receipt will be displayed. Please save it for future reference.
  8. Next the applicant has to pay the prescribed fee which is mentioned in “Fees” section of this page.
  9. After successful payment the application will be processed.
  10. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.


Note: PSA Serbilis uses the credit/debit/prepaid card and e-wallet payment facility of PesoPay. Unisys Managed Services Corporation will appear in the credit/debit/prepaid card and e-wallet payment statement and in the official receipt(s).



Apply In-Person:

  1. To apply for Delayed Birth Registration, the applicant has to approach the Philippines Statistics Authority’s civil registry office link.
    Note: The applicant has to click Field office and select the region from the drop-down menu.
  2. An application form will be provided to the applicant by the concerned officer. Or the applicant can download the application form from the following link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the concerned officer.
  7. The applicant has to pay the prescribed fee as requested by the authority.
  8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  9. An acknowledgment receipt will be provided. Please save it for future reference.
  10. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  11. This entire process can be completed within 15- 30 days or as per the timeline advised by the respective department authority.

Eligibility

Only the following are authorized to receive birth/marriage/death/CENOMAR certificates:

  • The owner himself or through a duly authorized representative;
  • His/her spouse, parent, direct descendants, guardian, or institution legally-in-charge of him/her, if minor;
  • The court or proper public official whenever necessary in administrative, judicial or other proceedings to determine the identity of a person;
  • In case of the person’s death, the nearest of kin.




Fees

  • To Request a Certificate via PSA Serbilis, the applicant has to pay the prescribed fee of PHP 330.00 per copy if the delivery destination is within the Philippines. The additional PHP 30.00 serves as the Documentary Stamp Tax
  • The applicant has to pay the prescribed fee of USD 20.30 per copy if the delivery destination is outside the Philippines.


Note: PSA Serbilis uses the credit/debit/prepaid card and e-wallet payment facility of PesoPay. Unisys Managed Services Corporation will appear in the credit/debit/prepaid card and e-wallet payment statement and in the official receipt(s).

Need for the Document

  • A Death Certificate is an official document setting forth particulars relating to a dead person, including the name of the individual, the date of birth and the date of death.
  • When requesting for death certificate, the interested party shall provide the following information to facilitate verification and issuance of certification.
  • Any applicant who had lost the original copy of the death certificate has to get a copy of the certificate from the PHILIPPINES STATISTICS AUTHORITY (PSA) which will be valid as same as the original one.
