1. To obtain a widowhood certificate you will to appear before a Notary Public.
2. You will need to complete an Application Form for Widowhood Certificate. A link is provided under 'Documents to Use' section, you may use the link to download the form.
3. In the application form you will need to declare that you are widow stating your deceased husband's name, nationality, date of death.
4. You will also need to declare that you haven't been married again.
5. Your application must be signed by you and two other witnesses.
6. Pay the application fee.
7. Once notarize, your certificate will be issued to you.
