1. To obtain a Disabled Person Card, you will need to apply at the nearest social development department in your area.
2. You will be given a letter to the Ministry of Health to apply for the medical report
3. With the help of the processing officer, you will need to complete the initial information in the form of (disabled person card request) which includes general and study information.
4. You will need to go to the Ministry of Health for the issuance of the medical report.
5. The processing officer will open the system and electronically enter all the information that you have provided. In the same time, he fill up a paper application form and forwards it to the manager.
6. After approval, you application will be saved and a card will be issued to you.

• Completed application form. The form can be obtained from the nearest directorate of Persons with disabilities or from the Ministry of Social Development's website.
• 2 personal photos
• Photocopy of the applicant's Identity Card or passport
• A medical report illustrating the type of disability and blood group
