A photocopy of a valid government issued photo ID by either the applicant or an individual attesting for the applicant is required for issuance of certificates.

  • State Issued Drivers license
  • State Issued Photo Identification card
  • US Passport
  • Government issued Military ID
  • US Bureau of Indian Affairs photo ID
  • Tribal photo ID
  • Oklahoma gun permit
  • Pilot license
  • A non US passport and I-94 card (VISA)
  • Consular Matricula card + 1 form of secondary ID

Secondary ID

(If using 2 forms of secondary ID the certificate MUST be mailed to the applicant! It cannot be presented in person. Mailing address must be on the application.)

  • Pay stub
  • W2
  • Vehicle registration with name and current address
  • Bank statement with name and current address
  • SSA record earnings statement
  • Copy of official police report related to the theft of ID
  • Utility bill with a current address
  • For any person under the age of 18, an affidavit signed by the parent or legal guardian
  • Photo identification card issued by one of the following: ?Oklahoma public, private, or parochial secondary school
    • Oklahoma institution of higher education
    • Oklahoma technology center school
  • Oklahoma employer
  • Oklahoma lifetime hunting or fishing license
  • Marriage certificate
  • Separation or divorce judgment
  • Health, life, or home insurance policy issued to the applicant
  • Automobile insurance policy or security verification form issued to the applicant
  • A valid U.S.D.O.T. health card, as required by 49 C.F.R. Part 391
  • An identification document issued by the United States Armed Services; one of the following: ?Military discharge (DD-214), unless specified not to be used for identification
    • Military identification card
    • Military dependent identification card
  • United States Bureau of Indian Affairs identification card or a Oklahoma tribal photo Identification card which identifies the person and includes all the following information: ?color photograph of the person
    • full legal name of the person
    • birth date of the person
    • signature of the person
    • signature of person who verifies records
    • tribal seal
  • Out-of-state driver license, if the issuing state participates in the REAL ID Act of 2005




EligibilityEdit

Oklahoma birth certificates are not open for public inspection. The person requesting the certificate must meet one of the following eligibility standards:

  • Be the subject of the record
  • Acting in such persons best interest (and authorized by the subject of the record)
  • Court order

All applicants must submit a copy of their government issued photo identification. By signing the request, you are indicating that you are the person who is the subject of the records, a court appointed legal guardian, a custodial guardian, or an authorized agent working in the best interest of the subject of record. Additional documentation may be required demonstrating the requestors authorization to obtain the birth record requested.

Expedited Service

Requests using the U.S. Postal Service's Express Mail Service, will allow processing of most request within 48 to 72 hours. Return of a certificate by the U.S. Postal Service's Express Mail Service will require that an envelope is attached with proper postage. Additional courier services such as Federal Express, UPS etc., will be accepted but items will not be returned through these services as account numbers will not be accepted since the Division has been advised by the major couriers, that as the sender we will be held res

Need for the DocumentEdit

You may need a birth certificate when enrolling in school, applying for a social security card or for certain legal proceedings. This can prove frustrating when you do not have a copy immediately on hand.
