Apply by Mail

1. To obtain a birth certificate, download and fill the application form with relevant details, use the following link to the Application Form.
2. The applicant will have to fill the application form with all relevant details.
3. The applicant will have to pay a fee of 10$ either by cheque or money, payable to the government of Nunavut, find fee-related information under the “Fees” section of this page.
4. Send the application form along with all relevant documents mentioned under the “Required Documents” section of this page by mail or courier to the following address: Nunavut Vital Statistics Department of Health Box 889 Rankin Inlet, NU X0C 0G0.
5. The applicant shall receive their birth certificate by mail within 10 to 15 days from the submission of the application.
