- To apply for a change of name in Nunavut, the applicant will have to submit a birth certificate and other id’s along with the application to the address mentioned under the “Office Locations and Contacts” section of this page.

- The applicant needs to fill out a form, find the application forms under the “Documents to Use” section of this page.

- Fill in all the details and submit the application form along with all the required documents as mentioned under the “Required Documents” section of this page.
- The applicant will have to pay a fee as mentioned under the “Fees to be paid” section of this page.
- The applicant will be informed when they have to go collect the form.
Required Documents
Translated copies of these documents (If they are not in English or French)
- Two certified copies of acceptable ID:
- Health care card
- SIN card
- Drivers license
- NTI enrollment card
- FAC
- Proof of name.
- Criminal record check or police information check.
- Copy of Birth certificate in the old name and/or copy of the baptismal certificate.
- Application forms adult.
- Application form child.

Eligibility
- The applicant must be 16 years of age or older.
- The applicant must be a resident of Nunavut.
Fees
- The applicant will have to pay a fee of 10$, this can be paid either by cheque or money order written out to the Nunavut Court of Justice.
Validity
- The document is valid forever and does not need to be renewed.

- The applicant will need to publish their change of name in the local gazette.

Need for the Document
- The document is mandatory if the applicant has/wants to legally change their name.
Other uses of the Document/Certificate
- The document can be used if the applicant wishes to change their name in other documents such as their passport, driver’s license, VAC card and so on.
