Apply In-Person

1. To apply for a change of name in Nunavut, the applicant will have to submit a birth certificate and other id’s along with the application to the address mentioned under the “Office Locations and Contacts” section of this page.

4. The applicant needs to fill out a form, find the application forms under the “Documents to Use” section of this page.

7. Fill in all the details and submit the application form along with all the required documents as mentioned under the “Required Documents” section of this page.
8. The applicant will have to pay a fee as mentioned under the “Fees to be paid” section of this page.
9. The applicant will be informed when they have to go collect the form.

Translated copies of these documents (If they are not in English or French)

• Two certified copies of acceptable ID:
  • Health care card
  • SIN card
  • Drivers license
  • NTI enrollment card
  • FAC
  • Proof of name.
• Criminal record check or police information check.
• Copy of Birth certificate in the old name and/or copy of the baptismal certificate.
• Application forms adult.
• Application form child.

• The applicant must be 16 years of age or older.
• The applicant must be a resident of Nunavut.

• The applicant will have to pay a fee of 10$, this can be paid either by cheque or money order written out to the Nunavut Court of Justice.

• The document is valid forever and does not need to be renewed.

• The applicant will need to publish their change of name in the local gazette.

• Original birth certificate
• Birth registration number
• Marital status
• Signature

• The document is mandatory if the applicant has/wants to legally change their name.
