1. Go to your regional Motor Vehicles Division to apply.
2. Fill out the declaration of lost or misplaced driver license. Declaration of Lost or Misplaced Driver License: http://www.edt.gov.nu.ca/apps/UPLOADS/fck/file/Lost_Misplaced_DL_Form.pdf
3. Submit the declaration form and pay the fees.

List down the documents required for successfully completing the procedure.

1. Proof of child birth from the Hospital.

For a lost or misplaced driver's license the fee is $15.40.
