1. Complete an Application for Death Certificate form, providing all relevant information with respect to the application. Application for Death Certificate Form: http://www.hss.gov.nt.ca/sites/default/files/application_for_birth_marriage_death_certificate.pdf
2. Pay by cheque or money order (payable to Government of the NWT). Note: Certificates come in paper (framing) size only, and are $20.00 for each certificate.
3. If a Restricted photocopy is required, there is a $30.00 charge per certificate. Note: A reason for requesting a Restricted Photocopy must be provided, and it will only be released at the discretion of the Registrar General of Vital Statistics.
4. Apply at the Health Services Administration, in person, by mail or by fax (if paying by credit card only).
