Apply In-Person:
- To apply for registering a death in person, the applicant has to visit the CivilRegistration and Vital Statistics department of National Population Commission of Nigeria.

- Visit the office and obtain an application form to register the death from the relevant department.
- Complete the application form with appropriate details as required in the provided fields respectively.
- Make sure that you have taken all the documents mentioned in the “required documents” section of this page, with you when you visit the office.
- After completed filling the application form, attest it with your signature and attach the documents required with it.
- Then, proceed to submit it to the relevant authority at the office.
- Also, pay the necessary fees (if any) at the relevant counter as directed by the person in-charge.
- The application form and attached documents will be verified by the concerned officials.
- Once everything has been done appropriately and the documents are sufficient for registration, the death of concerned person will be registered by the relevant authority.
- The applicant will be notified once when the document is ready and he/she can also check with the office they applied, for the status of application.

Eligibility
- Any person, whose death needs to be registered inside or outside Nigeria, is eligible for the registration.
- The applying person can be his/her family member or any responsible person.
- Both the applicant and the deceased must possess a valid proof of Identification.
Fees
- Usually, the service is free. But it may vary depending upon instance of the registration.

- Death Registration is valid forever.

Instructions
- Applicants are advised to complete the application form neatly and completely with absolute legibility and appropriateness to avoid rejections. Also, all the required documents must be furnished completely.
- Applicable proof of death can be a Certificate issued from the hospital where death has been occurred or from a Medical Practitioner who analyzed the corpse.
- In case if the death has been occurred in places other than hospitals, a doctor can be called for analysing or any local authority can be approached for a Permission letter.
- Deaths of foreigners occurring inside Nigeria should also be registered in the National Population Commission whereas deaths of Nigerian citizens occurring abroad should be registered with the Nigerian Embassy/Consulate in that country.

Need for the Document
- It is necessary register all the deaths of Nigerians occurring inside and outside Nigeria with the Governmental authority which is Civil Registration and Vital Statistics department of National Population Commission.
- The process of obtaining a death certificate is relatively straight forward if a person dies at a medical facility or at another location under known medical circumstances, or in an obvious accident.
- Please note that only one original death certificate will ever be issued from the National Population Commission of Nigeria.
- Standard practice in Nigeria is that all bodies are embalmed prior to transport or burial, usually within 24 hours of death when possible.
Other uses of the Document/Certificate
- A Death Certificate may be useful for claiming Insurance which the deceased person has during his/her lifetime.
- It may also be required for various compensation/subsidiary claiming processes and many other official verifications.
