Nigeria - Obtain a Certificate of Good Health


Procedure

Apply In-Person:

  1. To obtain a Certificate of Good Health in person, the applicant has to visit any Government/Private hospital nearest to his/her location.
  2. Nigeria Health Facility Registry (HFR) of Hospitals can be found in the following link: list of health facilities
  3. Visit the concerned health facility and obtain an application form getting registered with the hospital.
  4. The officials may require you to present a set of documents for scrutiny, which can be found in the “Required Documents” section of this page.
  5. Then, your identity will be verified and your name will be registered for further treatment.
  6. A separate file will be opened in your name and a hospital card will be issued to you as an acknowledgement.
  7. Further, you will be instructed to undergo some tests in the hospital lab itself or any other laboratories (as the case may be).
  8. Nigeria Health Facility Registry (HFR) of Laboratories can be found in the following link: Laboratories list
  9. Proceed as directed by them and complete the required tests. Then, you will have to collect the test reports from the laboratories and submit them in the health facility.
  10. Doctors over there will go through your test reports and issue you with a Certificate of Health, according to the test results.
