Apply In-Person:

1. To obtain a Burial Permit in person, the applicant has to visit the office of Birth and Death Registration or a funeral home in your locality.

3. Visit the office and obtain an application form for a Burial permit from the relevant department.
4. Make sure that you have all the required documents listed in the “Required documents” section of this page.
5. Complete the application form as required with necessary information and attest with your signature.
6. Then, attach the documents required with the completed form proceed to submit it to the concerned official at the Office for further verification and application processing.
7. Also pay the required fees for the application process at the office as directed by the officials.
8. The application and the documents attached will be forwarded to the concerned departments for their scrutiny.
9. After all the processes are completed, concerned officials will approve the application and proceed with the permit issuance process.
10. Once the document is ready to be collected, office will inform the applicant to collect it through any applicable means.

• Applicants are advised to complete the application form neatly and completely with absolute legibility and appropriateness to avoid rejections. Also, all the required documents must be furnished completely.
• The process of obtaining a death certificate is relatively straight forward if a person dies at a medical facility or at another location under known medical circumstances, or in an obvious accident.
• Please note that only one original death certificate will ever be issued from the National Population Commission of Nigeria.
• Standard practice in Nigeria is that all bodies are embalmed prior to transport or burial, usually within 24 hours of death when possible.
