Apply via Agent
Apply In-Person:
- To obtain a Burial Permit in person, the applicant has to visit the office of Birth and Death Registration or a funeral home in your locality.

- Visit the office and obtain an application form for a Burial permit from the relevant department.
- Make sure that you have all the required documents listed in the “Required documents” section of this page.
- Complete the application form as required with necessary information and attest with your signature.
- Then, attach the documents required with the completed form proceed to submit it to the concerned official at the Office for further verification and application processing.
- Also pay the required fees for the application process at the office as directed by the officials.
- The application and the documents attached will be forwarded to the concerned departments for their scrutiny.
- After all the processes are completed, concerned officials will approve the application and proceed with the permit issuance process.
- Once the document is ready to be collected, office will inform the applicant to collect it through any applicable means.

- The applying person can be a family member or close relative to the deceased.

Instructions
- Applicants are advised to complete the application form neatly and completely with absolute legibility and appropriateness to avoid rejections. Also, all the required documents must be furnished completely.
- The process of obtaining a death certificate is relatively straight forward if a person dies at a medical facility or at another location under known medical circumstances, or in an obvious accident.
- Please note that only one original death certificate will ever be issued from the National Population Commission of Nigeria.
- Standard practice in Nigeria is that all bodies are embalmed prior to transport or burial, usually within 24 hours of death when possible.
