2. Complete an application form. You can download a copy of the form using the link provided under the 'Documents to Use' section. You can print and complete the form at home and bring it with you when you visit the driver licensing office.
3. Present evidence of your identity.
4. Pay the replacement licence fee of $38.20.

Lost or stolen driver's licences must be replaced as soon as possible. Getting a replacement licence will automatically cancel your old licence and reduce the opportunity for others to use it fraudulently. You can also apply to replace your licence at any time if it's damaged or you've changed your name.

In case you found your old licence:

You will need to hand in your old licence to any driver licensing agent or mail it to the driver licensing office in Palmerston North. This ensures that it can't be used fraudulently.
