Forms are available at other locations throughout the country (i.e. bottle stores, tertiary providers, bars, etc), you will however need to take your completed application form to a NZ Post outlet to get them checked and processed.

• Proof of address
• An identifier statement (if required)
• Completed application form

To Apply for Your 18+ Card You will need to be at least 18 years of age

Pay a fee of $20

18+ Card is valid for 10 years from date of issue.

Please attach documents which can be used by people who would like to follow this procedure.

You will need one form of Primary ID either:

• Passport (NZ or overseas, current or have expired in the two years prior to applying)
• Full Original Birth Certificate
• NZ Photo Driver Licence
• Firearms Licence
  

If you dont have one of these forms of ID you will not be able to apply for an 18+ Card

• Supply a passport photograph
• Provide a primary identification document
• Provide proof of address
• Provide an identifier statement (if required)
• Complete the application form

The Hospitality NZ 18+ card is an approved photographic Evidence of Age Document under the Sale of Liquor Act, and along with photo driver licence or passport is a means by which young people can establish that they are over 18 and entitled to purchase alcohol.
