2. Present evidence of your name change. If you have changed your name several times, the licensing agent may request to see all of your original certificates and name change documents. This is so they can track the name on your current licence through to the name you want to appear on the new licence.
3. Pay the application fee:
   1. Replacement of license costs $38.20
   2. If you are applying for change of name when your current licence or endorsement expires. The normal licence or endorsement renewal fee will apply.
   3. If you are applying for other licence classes or endorsements at the same time. The normal licence or endorsement application fee will apply.
