1. Visit a NSW registry office.
2. Complete a Replacement Application form 1018, available to download online. Replacement Application Form: http://www.rta.nsw.gov.au/publicationsstatisticsforms/downloads/45070182.pdf
3. Produce the required documents stated below.

Original copy of the following:

• A marriage certificate # (Commemorative Certificates are not accepted).
• A divorce decree (bearing the name being reverted to).
• A deed poll * registered with the relevant authority.
• A change of name certificate issued by the Registry of Births, Deaths and Marriages.
• A birth certificate showing the name at birth and the new name (Commemorative Certificates are not acceptable).
• A document showing evidence of change of name registered in the Land Titles Office.
• a Full proof of identity (POI) document showing the 'original' name; or a List 1 POI document showing the 'original' name and a List 2 POI document showing the 'original' or 'new' name.

• Your identity and change of name documents should show a clear link between your names.
• Documents not in English must be accompanied by an acceptable translation.
• For NSW Marriage Certificates, Roads and Maritime Services only accepts those issued by the NSW Registry of Births, Deaths and Marriages to support a change of name.
