• You may print out, fill and mail (do not email) the death search application form.

• You may send a letter with the following information

1. The decedents name.
2. Date of death.
3. City of death. Include county if known.
4. The decedent's Social Security Number, if known.
5. The name of the mortuary in charge of final arrangements.
6. Requestor's relationship to the person on the death certificate.
7. The purpose for which you are requesting the death certificate.
8. Your name and signature.
9. Your mailing address.

• If you receive your mail at a Post Office Box, please include a physical street address for our internal files.

In-Person

1. Applicant must present current government issued picture identification and complete a Search Application. The search fee for a death certificate is $ 5.00 per certificate.
2. Counter Service through the Santa Fe State Office: Monday through Friday 8:30 am to 4:00 pm. Please note that on the 2nd Thursday of each month, the Counter is closed 8:30 am to 9:00 am.

• State law restricts access to death certificates to the deceased registrant's (also called decedent) IMMEDIATE FAMILY members or those who represent tangible proof of legal interest in the requested record.
  
• Immediate Family means any of the following: mother, father, sibling, child, current spouse, or maternal or paternal grandparent. Paternal grandparent is eligible if father is listed on the vital record.

• The fee for a death certificate is $ 5 per certified copy. Please make your certified check or money order payable to New Mexico Vital Records.
• Do not send cash.
