New Hampshire - Apply for Duplicate Driver License


Procedure

In Person You can apply for a duplicate driver license by visiting any DMV office with the following:

  • An application for your license type: Driver License, Commercial Driver License or Non-Driver Identification.
  • Positive proof of identification.
  • Duplicate fee of $10.00.

You will receive a 60-day, temporary paper license. Your permanent license or non-driver identification card will be mailed to your New Hampshire mailing address.

By Mail You can apply for a duplicate driver license by mail if you are temporarily away from New Hampshire. To apply please send the following

  • A completed and signed application for your license type: Driver License or Commercial Driver License.
  • An Out of State Residence form providing an out-of-state address.
  • A check or money order for $10.00 made out to "State of NHDMV."

And mail your application, fee and of state residence form to:

The DMV will mail a temporary duplicate license which will be valid for six (6) months to your out-of-state address. You are required to go to a DMV office when you return to New Hampshire to complete the duplicate license process.

Instructions

  • If your driver license is lost or stolen you can apply for a duplicate driver license by visiting any DMV office and submit the all required documents and pay required fee.
  • If you are temporarily away from New Hampshire and your license is lost or stolen, you may apply for a temporary duplicate license by mail. If your image is not on file at the DMV, you will be unable to get a duplicate license by mail.

Proof of Identification

  • You are required to provide positive proof of identification for every driver licensing transaction. If your image is on file at the DMV, in most cases, you may use that as positive proof of identification. If you previously requested to have your image removed from the DMV computer system, you will be required to provide two forms of identification and proof of New Hampshire residency to get a duplicate license or to renew a license.

When you drive a motor vehicle in New Hampshire, you are required by law to have a driver license with you. If your license is lost, stolen or destroyed you must apply for a duplicate license.
