  2. Fill and complete the form.
  3. Mail the completed application form to:

  1. Complete Application Form, Verify and Confirm You must complete, verify and confirm all required fields of the application form in order to proceed.
  2. Purchase Information You will confirm the type and number of birth certificates to be purchased.
  3. Payment Information You will be asked for your credit card or debit card information to complete the payment transaction.
  4. Transaction Confirmation - The results of your transaction will be confirmed, including details of your payment, which may be printed for your records.

  • name of Funeral Home(if known)
  • relationship to person named on certificate
  • reason why certificate is required
  • daytime phone number and return address.

The Vital Statistics Branch registers deaths and provides death certificates of deaths occurring in the Province of New Brunswick.
