1. Completing a donor registration form: You can register as an organ or tissue donor by completing a donor registration form, either online or you can also download a donor registration form (in Dutch) from the Donor Register website. You may also request for the form by calling 0900 - 821 2166 (calls charged at local rate in the Netherlands). The forms are also often available from your doctor, pharmacy or the Population Affairs Department (burgerzaken) of your local town hall.
2. Registering your decision:
   1. When you join the Donor Register, you can choose from four possible options: (1) Yes, I give consent. (2) No, I do not give consent. (3) I leave the choice to my nearest relatives. (4) I leave the choice to a specified person.
   2. You will also can choose what organs or tissue you would like to donate. Organs that can be transplanted are the heart, kidneys, liver, lungs, pancreas and intestines. The skin, bone tissue (including tendons and cartilage), cornea, heart valves and blood vessels are transplantable forms of tissue.

Changing your decision

You can change your decision at any time by completing a new form. To register or change your donor registration online, you must have a DigiD.

You can register as an organ or tissue donor by completing a donor registration form, donor forms are distributed automatically to:

• young people when they turn 18
• (in the case of most municipal authorities) people collecting a new travel document or passport
• all newcomers to the Netherlands.
