Muntinlupa - Apply for Occupational Permit

  1. An individual Mayor’s Permit is required for workers or employees (temporary or permanent) who are working within the jurisdiction of the City. This said permit is called Occupational Permit. Here's how you can apply for one. First you will need to obtain a community tax certificate. You need a valid community tax certificate number (for the current year) to complete the application form.
  2. Next, you will need to go to clinic and have a chest X-Ray, urinalysis and fecalysis.
  3. Complete the Occupational Permit Application Form. A link is provided under 'Documents to Use' section which you can click to download the form. Do not forget to sign the form and attach a 1x1 picture of you.
  4. Submit the completed form and all the lab results to the Business Permit and Licensing Office of the Muntinlupa City Government.
  5. Pay the application fee.
  6. The BPLO officer will inform you when you can pick up your occupation permit.

  • Medical Laboratory Examination Results:
    • Chest X-Ray (1 year validity)
    • Urinalysis (6 months validity)
    • Fecalysis (6 months validity)
  • Community Tax Certificate (Cedula) (current year)
  • 1 pc. 1×1 I.D. Picture
  • Other requirement that you might need:
    • Parents/Guardian’s Consent with photocopy of their ID (for 17 years old and below)
