4. Pay the fee.

Loss of family book

• a statement issued by the competent loss or presentation booklet deteriorated authority
• a request to the registrar who established the original booklet to obtain a new copy

Loss or damage to the identity booklet or the Vital

• a statement issued by the competent loss or presentation booklet deteriorated authority
• a request to obtain a copy sent to the registrar of the place of birth
• a full copy of the extract of the birth certificate of the husband or the wife
• a certified copy of the marriage certificate or other document evidencing said marriage copy
• a copy of the national identity of the husband or wife of the map
