2. Bring the required documents.
3. Present the documents.

• Application (form AJC200)
• Status of taxpayer or imposition notices
• Payment receipts
• Other supporting documents of the claim.

• The application can be submitted to Reception and coordination office at the regional or district directorate of the tax domicile or main business place of the taxpayer.
• Tax clearance can be issued at the above mentioned reception and coordination office or collection at the tax domicile or main business place of the taxpayer.

e.g.
1. Date of Birth.
2. City or County of Birth.

A Claim is a request or demand for payment in accordance with an insurance policy, a workers' compensation law, etc.
