2. Go to the Civil Service Office submit your request to obtain a duplicate Family Record Book and do not forget to bring your ID card.
3. Pay for the booklet.
4. If you would like to have the duplicate Family Record Book sent to you, get in touch with the Civil Service Office (use the contact information provided below).
