1. Go to your designated driver license office
2. Exhibit the following : Proof of identity, residency, Social security number
3. Make the necessary payments for this duplicate license fee.
4. If the lost license will expire within the next 6 months, the license office will instead allow you to renew your license instead of obtaining a duplicate license.

• Complete the Form 4317- Mail in Driver License Application.
• Attached the following: Proof of identity, legal residency, and lawful presence.
• Include the necessary fees in your mail
